A GP Management Plan (GPMP) can help people with chronic medical conditions by providing an organised approach to care. A GPMP is a plan of action you have agreed to with your GP. This plan identifies your health and care needs, sets out the services provided, and lists the actions you can take to help manage your condition.
To be eligible for a GPMP, you must have a chronic or terminal medical condition with or without complex care needs.
A chronic health condition is one that has been present for 6 months or more and may include, but is not limited to:
- Asthma
- Cancer
- Cardiovascular illness
- Diabetes mellitus
- Musculoskeletal conditions
- Stroke.
All patients with a chronic disease are eligible for a bulk billed GPMP, and often a Team Care Arrangement (TCA), which enables access to up to 5 Medicare subsidised allied health consultations. To ensure continuity of care, it is important that you continue to visit your usual GP for your initial plan and any reviews of your GPMP plan.
Team Care Arrangements (TCAs)
If you have a chronic medical condition and complex care needs requiring multidisciplinary care, your GP may also develop Team Care Arrangements (TCAs). TCAs will require you to collaborate with at least two other health care providers who will give ongoing treatment or services to help you.
